'Per condition' cover means that each condition is covered up to a certain amount.
When an insurer says 'condition', we mean any illness, injury, sickness or disease (it is worth bearing in mind that the word 'condition' doesn't just apply to things that are ongoing).
'Per condition' policies provide a maximum benefit per condition. Once the condition limit has been reached, that condition will be excluded going forward.
This would mean that if an ongoing illness (such as diabetes) were to arise, you could claim up to the vet fee limit for that condition. However, once the limit runs out, you would not be able to claim for that condition any more.
